As a non-profit homecare provider, SummitWest Care believes that people deserve to heal at home. Our experienced nurses and homecare specialists provide services to both private insurance patients as well as Medicaid, Medicare and other underserved individuals.
We are committed to filling the gaps in the homecare community and providing services where the demand is heavy in the Grand Valley.
SummitWest Care does not currently provide Speech Therapy and Infusion Services.

Services
We are proud to provide both medical and non-medical homecare services to all ages of our community. SummitWest Care is based in Grand Junction, CO and proudly serves Mesa, Gunnison, Pitkin, Eagle and Garfield counties. We serve those who are ill, temporarily disabled and those who require long-term care. All nurses and therapists are provided at the direction of an established physician’s plan of care.
Newborn and
Pediatric Homecare
SummitWest Care is the only provider of pediatric and newborn homecare on the Western Slope of Colorado. Parents/Guardians who are concerned about bringing your infant/child home and may need specialized homecare assistance should contact us immediately. Our expert nursing team can help you decide if newborn and pediatric homecare services are right for you.
Telehealth
Services
SummitWest Care is proud to provide high-tech, personalized tele-health solutions for both our clients and referrals from our partnership with Community Hospital. Our telehealth services reduce the cost of services and increase access to healthcare by featuring continuous Bluetooth biometric monitoring, medication reminders, wound imaging and customized care plans.

The nurses helped me daily to clean, drain and bandage my incision that was infected. They kept me talking and laughing through the most painful times. I greatly appreciate all they did to aid me in my recovery! I could not have done it without their help! I would recommend these girls to anyone needing that extra TLC to aid in recovery!
Elizabeth had Monday off and was going to see me on Tuesday. I had a meeting on Tuesday at an assisted living place and wanted to look nice, so Elizabeth came over on her day off to help me get a bath. It’s not an easy task because I have MS, but she was so caring and gracious. I am so thankful for her and want everyone else to know how amazing she is. She takes such good care of me. I don’t know what grand junction would do without summit west care. They are an exceptional support system. They are there no matter what the problem is. My husband was on a wound vac. I would not have made it through this without their support. We are extremely lucky to have them in grand junction.

With telehealth in my home, utilizing the education videos, it taught me the importance of monitoring my weight and blood pressure on a daily basis helping to keep my chronic condition controlled. I like the security of knowing that the Telehealth nurse is always there to talk with via phone or video call for any questions or concerns.

SummitWest Care provides language assistance services to individuals with limited English proficiency through LANGUAGE LINE.
