SummitWest Care’s highly trained, compassionate team delivers a full spectrum of healthcare services, offering both medical nursing services and non-medical homecare services to the Grand Valley and beyond.  Our team is experienced with patients of all ages; whether you need chronic illness management, pediatric homecare, or housekeeping and general assistance, we promise all our clients the best possible care and support.

Non-Medical Care and Support

Our program will train and educate family members and friends to care for their loved ones. If you have Medicaid, you may qualify for this program. The program is designed to provide care, training & resources at home for the elderly, disabled, or children with disabilities. We can train and educate family members or friends to care for the client while they work as an employee of SummitWest Care. If no family member or friend is available, we will train a caregiver for the patient and their specific needs.

Parent CNA Program

Caring for a child with medical needs can be a full-time responsibility. Colorado’s Parent CNA Program allows eligible parents to become certified as a Certified Nursing Assistant (CNA) and receive payment through Medicaid for providing care to their own child at home.

SummitWest Care helps guide families through the process of becoming a CNA caregiver for their child, while ensuring children receive safe, high-quality care under the supervision of our experienced nursing team.

Specialized Physical/Occupational Therapy

Our team will collaborate with patients to design a personal treatment plan to ensure optimal outcomes. This offers cost-effective treatment that improves mobility, relieves pain, reduces the need for surgery and/or medications, and allows patients to participate in their recovery at home.

When life events present health challenges, we are here to assist you and your family so that you can maintain independence in your own home and feel safe. Whether you’re recovering from a hospital stay or receiving other medical treatment, our expert clinicians are ready to respond to your health care needs.

We believe that remaining at home is an important part of health recovery. For that reason, we also offer Mobile-Mate (information below), a personal electronic monitoring system that provides immediate emergency notification 24 hours a day.
