How to receive Medical Home Healthcare Services

Insurances accepted: Medicare part A/B. Private Insurances, Private Pay

1.You are under the care of a Doctor.

Having an established primary care doctor is vital to receive home health care. Doctors follow care, approve orders and work side by side with us to ensure you’re getting the best possible care.

2. The doctor certifies that you need:

  • Skilled Nursing and/or Therapy Services
  • Temporarily Homebound

Medicare requires homebound status.

To be homebound means you have trouble leaving your home without help because of an illness or injury, or leaving your home isn’t recommended because of your condition.

3. Documented Face-to-Face encounter note from your doctor.

SummitWest Care will work with your doctor to get you the home healthcare services you need. A face-to-face encounter must occur. This means that you have seen your doctor and they have documented the reason homecare is needed.

How to receive Non-Medical Home Care Services

Services: Homemaking, Personal Care Providers, In-Home Support Services

Accepted Insurances: Medicaid and Private Pay

We work with the Department of Human Services, Physician Offices and family members.

Private pay is always accepted.
