Newborn and Pediatric HomeCare

SummitWest Care is the only provider of Newborn and Pediatric homecare on the Western Slope of Colorado. Our nurses are trained to work with the needs of the family and specialize in pediatric care services. If your child is experiencing breathing problems, born prematurely, as congenital conditions, jaundice or neurological disorders there is help available for your family.

Telehealth & Remote Patient Monitoring

Our Telehealth Program increases access to care with more personal attention between in-person visits. Our state-of-the-art Bluetooth technologies sends real-time data and messages back and forth between clinician and patient. Our easy-to-use technology has audio and visual conferencing capabilities, vital tracking, disease-specific education, symptom survey’s and daily reminders.

Medical Homecare Services

As a non-profit, home healthcare agency, we are patient focused and develop programs that meet the needs of our patients. Professional, experienced, in-home nurses are available for adult and senior care, newborn and pediatric care as well as physical and occupational therapy for recovery and pain management. In addition, SummitWest Care offers a low vision program for those who need additional training and adaptive devices as well as a swallowing program for patients with swallowing disorders.

Non-Medical Care and Support

Our program will train and educate family members and friends to care for their loved ones. If you have Medicaid you may qualify for this program. The program is designed to provide care, training & resources at home for elderly, disabled or children with disabilities. We can train and educate family members or friends to care for the client while they work as an employee of SummitWest Care. If no family member or friend is available, we will train a caregiver for the patient and their specific needs.

Complete LSVT BIG® Therapy for Parkinson’s Disease and other Neurological Conditions

LSVT BIG trains people with Parkinson’s disease (PD) to use their body more normally.  People living with Parkinson’s disease or other neurological conditions often move differently, with gestures and actions that become smaller and slower. LSVT BIG effectively trains improved movements for any activity, whether “small motor” tasks like buttoning a shirt or “large motor” tasks like getting up from sofa or chair or maintaining balance while walking.

We believe that remaining at home is an important part of health recovery. For that reason, we also offer Mobile-Mate (information below), a personal electronic monitoring system that provides immediate emergency notification 24 hours a day.

Life Care Planning

SummitWest Care helps provide solutions to crisis situations. Unplanned events in life are difficult to deal with. We can help. Our Social Workers are trained to be the pivotal link between independent in-home care and our clients. Our Social Workers can mediate and offer solutions to crisis situations, as well as teach independent behavioral skills. Our Life Care Planning program empowers the elderly generation to make decisions for themselves. We evaluate individual needs, provide options to develop a life care and crisis prevention plan and provide and teach advocacy services.
